unable to use key shortcut to ignore debuff

yes, I disabled all add ons except vuhdo and vuhdo options and I was still unable to put a debuff on the ignore list

Summary of the issue:
When I am trying to add a debuff to the ignore list using the keyboard shortcut (currently set to 'alt+ctrl+shift' from the vuhdo health bars, it does not take the debuff off at all. It continues to appear. Adding the spell to the debuff list works, however.
Expected behaviour:
The debuff would disappear from the bars and be added to the ignore list
Actual behaviour:
Nothing seems to change when using the shortcut key.

I'm not really sure. This used to work for me in BFA. Since then, I have added more add ons, however even when disabling all other add ons the keyboard shortcut does not seem to work. Specifically, I have tried to use this keyboard shortcut while in mythic+ dungeons and torghast.
